How to RegisterTo apply:
Send us some information about yourself, via email or email attachment (please put "HTA on the Net" in the subject line), to htaprof@uvic.ca . Your CV or resume will contain most of the information we need.
Information required:
- full name
- mailing address
- email address
- home and work phone and fax
- occupation
- employer (or whether you are self-employed)
- academic and work background
- why you're interested in the HTA course
- who would be paying your course fees
Payment information:
Payment is not required until we advise you (by email) that youare registered. Email is not secure, so DO NOT send credit cardinformation in your email.Course fees must be paid at time of registration. (The bestprocedure for registration is fax, using a credit card.) Therewill be a $50 processing fee for cancellations two weeks or lessbefore the scheduled start date of the course. Fees arenonrefundable once the course has started.The basic course fee is C$470. In addition, participants willneed to:
provide their own Internet access;- purchase a course pack of assigned readings (including one video) from UVic; and
- arrange access to the Cochrane Database of Systematic Reviews and the latest release of American College of Physicians' Best Evidence (combining ACP Journal Club and Evidence-Based Medicine). Enrolled participants can register for the Cochrane and ACP databases at student rates (approximate total C$225).
